For the recovering alcoholic or addict who is returning home and to the work place, a structured environment with a defined continuing care plan is necessary to optimize this successful transition.

Focus Healthcare of Tennessee's Continuing Care Plan may include:

• A professional monitor
• Random urine drug screen
• Daily 12-Step meetings
• Identification of a home group
• Having a sponsor
• Having a continuous care group
• A referral for individual therapy
• An action plan for relapse
• Quarterly monitoring reports (when required)
• Joining the Focus Healthcare Alumni
